The hallmark is Birmingham 1999: anchor for Birmingham, lion passant for sterling silver and date letter Z. The maker/sponsor, C.H, is not known to me. Note that the hallmark has been applied by a laser rather than the more traditional punch method.
Yes you are perfectly correct - it is 1974, not 1999.
